Tattoo Appointment Request Form

Please use this form to submit your tattoo request and allow for 1-3 weeks for an e-mail correspondence.
We do our best to get through all e-mails in a timely manner, but due to how busy our artists are this is not always possible. If you aren’t contacted back within 4 weeks, unfortunately it is probably not something our artists want to do at this time.  Thank you for your patience and understanding.


  • All artists at Beloved require a NON-REFUNDABLE DEPOSIT to book an appointment (which will come off the price of the tattoo when it is completed).
  • Cancellation Policy:
    • We ask that you give 7 days notice if you need to reschedule/cancel an appointment. Failure to provide this notice will result in loss of deposit.
    • All cancellations/rescheduling must be handled via the shop phone, we will no longer consider any other form of communication (Facebook, Instagram, E-mail, etc.). We ask for your understanding, our artists work extremely hard for their clients and deserve that respect in return.
  • We offer free consultations after the tattoo request has been reviewed by the artist. If you have any reference pictures, please feel free to attach them using the file upload section.
  • We don’t provide quotes via email or over the phone.
  • Please eat a substantial meal before your appointment and drink plenty of water! If you plan to bring a guest to accompany you for your appointment please limit it to 1 person (we only allow 1 additional person in the artist area while getting tattooed)​
  • Walk in’s: If you’re looking to walk in, please call the shop and we will let you know what we have available.
  • If you are looking for a cover-up, please make sure to attach a *current photo* of the tattoo in the file upload section.
